feat: delivery and read receipts for a message's sender (#24)

A member who sends from Link can now be told how far a message has got:
how many recipients' handsets have it, and how many have read it.

- Recipient rows gain received_at (schema version 3). It is written only
  by a handset that has opened the message, never inferred from a fetch:
  the poll is strictly exclusive, so a message that arrived by push is
  never fetched and a server counting fetches would miss the fastest
  deliveries.
- POST /messages/received takes the ids a handset has opened. Ids not
  addressed to its member are ignored and answered with the same "ok",
  so a batch cannot be used to probe for other people's messages. The
  first handset's time is kept.
- Marking a message read marks it received too, where it was not, so a
  lost acknowledgement does not leave the sender being told otherwise.
- GET /messages/receipts answers recipients, received and read counts
  for the member's own sent messages, from one grouped query. Ids they
  did not send are left out rather than refused. Counts only: no subject,
  no body, and not who has read it.
