Third type of Phase 2 group 5c (#1467).

    core.hpp       55,977 lines / 254 headers / 0 I/O-family
    bisection.hpp  72,698 lines / 317 headers / 4 I/O-family  (was 72,699 / 315 / 4)

Zero I/O-family headers is met. Under-45,000 lines is not, and reporting the
number rather than moving the bar: bisection stores its encoding in a
blockbinary, and internal/blockbinary/blockbinary.hpp is 33,257 lines on its
own (0 I/O, layered in Phase N); <cmath>, <string> and <algorithm> make up most
of the rest.

The smallest split in the group

bisection_impl.hpp included <sstream> and <iomanip> and used neither. Its one
stream use was a two-line operator<< that streams the decoded double; that
moves to iostream.hpp, which includes only core.hpp. The impl keeps <iosfwd>
and <string> (the to_binary friend declaration names std::string).

core.hpp also takes generators.hpp: the generator/refinement pairs and the
bisection_unary / _elias_gamma / _elias_delta / _posit / _fibonacci / _lns
aliases are how every caller names a bisection type, and they are pure
computation.

Self-containment, each found by compiling the header alone:

- generators.hpp used uint64_t with no <cstdint>, and its aliases name
  bisection<> with no declaration of it: 26 errors standalone, on main too. It
  now includes <cstdint> and bisection_fwd.hpp.
- numeric_limits.hpp included nothing; now <limits> and the fwd header. No
  umbrella cycle.
- manipulators.hpp and attributes.hpp now include core.hpp.

BISECTION_THROW_ARITHMETIC_EXCEPTION defaults in bisection_impl.hpp (#1436).
Nothing reads it yet; it moves so the shape matches the other cores.

Verification

- Layer gates, gcc 13.3 and clang, -Wall -Wpedantic: core.hpp alone, core +
  manipulators, core + iostream and core + attributes each compile, link and
  run, over ... (continued)
