chore: require Elixir 1.19, matching what CI verifies

The declared floor was 1.15, raised from 1.14 only because ex_doc 0.40 needs
it - not because anything verified it. When the CI matrix came down to Elixir
1.19 and 1.20, the declaration kept claiming support for 1.15 to 1.18 that
nothing tested, so a regression on those versions would have gone unnoticed.

The floor now states what is actually supported, and CONTRIBUTING.md and the
changelog agree: Elixir 1.19 or later, OTP 28 or later.

This is a breaking change for anyone on Elixir 1.15 to 1.18. Nothing in the
library requires 1.19 - it compiles and passes on older versions - so if that
support matters, the alternative is to restore a 1.15 leg to the CI matrix
rather than to widen the claim again.

Verified: deps resolve, formatting, compile --warnings-as-errors, and 389
tests passing.
