Bump @actions/github to v8 and @vercel/ncc to v0.45 (#153)

@actions/github v9 dropped CommonJS support entirely (ESM-only
exports map), which is what broke ncc bundling in #129/#128/#126.
Capping at v8.0.1, the last CJS-compatible major, avoids that.

v8's bundled octokit REST plugin also drops the flat octokit.repos.x/
octokit.pulls.x namespace in favor of octokit.rest.repos.x/
octokit.rest.pulls.x, so src/github.js and its test mocks are updated
to match.

Bundling the newer octokit code (which uses private class fields)
also requires bumping @vercel/ncc from 0.33.3 to 0.45.0 -- the old
version can't parse that syntax.
