feat(titlebar): group the most-run custom commands on top of the popover (#584)

once keymap.conf holds more than five commands, up to five previously run commands with the highest all-time counts lead the popover above a separator, the rest below. Counts decide only membership: both groups keep file order, so use changes which commands sit on top and nothing else. Every run counts, chord, palette or popover, recorded by the runner into custom-command-usage.json in the state directory. A second command line with a name already taken is skipped with a diagnostic, since the name is the count's key.
