fix: put the sealed envelope in the APNs payload explicitly

Link's iOS head now opens pushed messages itself, and everything it needs
to do that is in `k` and `p`. Those travelled only in the top-level data
block.

FCM does merge that block into the APNs payload as custom keys, so the
app delegate would have found them either way. But that is a documented
behaviour of FCM's rather than a guarantee of ours, and the failure if it
ever stopped is the quietest available: a silent push carries no alert,
so a handset that received one with no fields in it would do nothing at
all — no error, no notification, and the message waiting for the next
poll. Naming the envelope once and sending it in both places costs a line
and removes the dependency.

Reach's transport was given the same treatment in the same pass, for the
same reason.

Two tests, neither of which existed: that the envelope reaches the APNs
payload with the aps dictionary intact beside it, and that the push is
still silent — priority 5, push type background, no alert. The second one
guards a constraint of APNs rather than a preference: it refuses priority
10 for a payload with content-available and no alert, and the reason
there is no alert is that the server cannot read the message it would
have to describe.
