refactor: type factories stop throwing for C errors (#316)

Factories and record completion used to throw invalid_argument for C
constraints (void arrays, overflow, incomplete members, duplicate names,
illegal bit-fields). Six catch bridges turned those into diagnostics.

Factories now construct even when C-invalid (overflow sizeBytes=0;
negative count stays logic_error). completeStructure / completeUnion /
relayoutFromMemberSpecs return const char* and do not assign the live
body on failure. Diagnosis is arrayTypeError on the constructed Type
plus Declarator::arrayConstraintError for unadjusted formals.

Name counters __L / L$str / L$cl move from process globals onto
SymbolTable.
