test: drive the cache clock instead of sleeping, and cover the untested paths

The elements-fetcher tests waited real milliseconds for entries to expire and
for background refreshes to settle, and one of them failed under full-suite
load when a 10 ms settle was not enough. lru-cache reads its TTL clock through
performance.now, so the tests now offset that clock to age entries and flush
promise chains to settle refreshes; overlap between requests is arranged with
a deferred upstream call. Nothing in them depends on machine speed any more,
and the one action that lived inside an assertion moved to a setup block.

Also covered: the marketplace fetcher's single-page path, its filter mapping
and malformed-page rejection; the smart-wearable and Ethereum item filters;
the cache key per page; and, for profiles, owned and unowned wearables and
emotes by token, the If-Modified-Since short-circuit and the entity-fetch
failure path.
