docs: record the cold cache, the Pulse deadline and the non-array 502

Round-2 review, untested claim 5: the report claimed the cold-cache-after-a-
redeploy behaviour was documented in the README and it was not. It now is — the
proxy cache is process-local, so the first /hot-scenes request of a fresh
process always pays the upstream call or its 503 — together with the trade-off
that a stale serve costs the full 5 s deadline, the new 5 s deadline on the two
Pulse reads, and the non-array 2xx now answering 502.
