docs: describe the amended /hot-scenes shape and the Pulse /realms read

openapi keeps `realms` required on HotSceneInfo, adds creator, projectId and
description as optional, replaces the 200 example with an entry the code can
actually produce (the contract pack's Genesis Plaza entry plus its realms
distribution), and documents both 503 bodies the proxy can answer: the
gatekeeper's `{"ok":false,"error":"warming"}` passed through verbatim, and
realm-provider's own `{"error"}` when the gatekeeper could not be reached.

README, .env.default and the agent context now say the main realm count
comes from PULSE_URL/realms (the `main` entry, Genesis City only), that both
pulse URLs are required at startup, and what the proxy adds, caches, bounds
and flags.
