Open every recording with a chapter (#41)

The news API answers 404 until a topic goes live, and recording starts
about four minutes before the show while the news part begins 45 minutes
to an hour and a half in. fetchInitialTopic treated that as a failure and
added nothing, so the Вступление chapter it has for exactly this case
never appeared and players showed no chapter at all for the opening.

Every one of the five recordings on the live server is affected: the
first chapter sits at 1h09m, 45m, 1h40m, 1h02m and 1h13m, leaving 24% to
45% of each file unchaptered, and none of them contains Вступление.

An initial topic that cannot be determined, whether the call failed or
there is simply no active topic, now opens the recording with the intro
chapter, and the first real topic starts its own when it arrives.
