Document and test arbitrary-start sample numbering (vp712, ...)

Generation counts from wherever first_sample starts and resolution
matches numbers as spoken; ordinals ('1' for the first of vp712..)
stay refused as ambiguous. Behavior was already correct - this pins it
with tests and states it in the docstrings.
