refactor(colors): one colour model — oklch palette, owned by the palette, emitted in the `colors` layer (#1068)

* fix(colors): emit the palette in the `colors` cascade layer

The palette was merged into `$root-tokens`, so every colour token landed in
`@layer root` even though the layer declaration advertises a `colors` layer
below it. An override written where the docs point — `@layer colors { :root {
--cui-blue-500: … } }` — lost to `root` and did nothing.

The palette now gets its own `:root, :host` block in `@layer colors`, built
from a new overridable `$color-tokens` map, so an override in that layer wins
by source order. Token names and values are unchanged in all three bundles
that carry them; the extra rule costs 43 bytes uncompressed.

`$color-tokens` is assembled straight into a private map instead of through a
`color-tokens()` function and a same-named mixin that only wrapped it; the
scale generator stays a function because _theme.scss calls it per theme colour.

* refactor(colors): the palette owns the theme colours

`_theme.scss` read Sass palette values and generated a 100–900 scale per theme
colour, so it had to `@use "colors"`. That put the palette in every bundle
loading the theme for its maps — `coreui-grid.css` grew by 117 tokens and
7.6 kB when the `:root` block sat next to the map — and left two scale
generators in the tree.

`$primary`, `$success`, `$info`, `$warning` and `$danger` are palette colours
now, so their scales come from the one generator in `_colors.scss` and the
theme keeps only its slots. `secondary` and `inverse` are built from the grays,
so they stay with the theme and lose a 100–900 scale nothing read: 18 tokens
off the output. Slot values that were Sass literals (`contrast`, `$link-color`,
the border- and shadow-colour utilities) now read the colour's token, so an
override at run time reaches them.

The `:root` block moves next to the palette it emits. Selectors are unchanged,
