* feat(trainer): add confirm gate to update_training_job

- Add confirmed parameter (default False) and PreviewResponse to update_training_job
- Place confirm gate after persona and MCP ownership checks
- Update tool description and instruction sections to document confirmed=True
- Add unit tests for validation, preview, ownership gating, and execution
- Update E2E confirm gate and lifecycle tests for update_training_job
- Update schema snapshot baseline
- Remove legacy exception references in README, AGENTS.md, and CONVENTIONS.md

- Add delete_training_job 404, non-admin ownership, and namespace policy tests
