Restore per-line timestamps in inlined logs; add a per-country header

Restores the original timestamp in _render_task_log()'s rendered lines
(reverting the previous commit's removal): it's the real time each line
was actually logged, unlike the outer JSON wrapper's timestamp on the
summarize_country_results line it's inlined into, which just reflects
whenever that task happened to run - the two aren't redundant, the inner
one is the more meaningful of the two.

Also replaces the "--- X (map_index=N) full log: <path> ---" header
before each country's inlined content with a plainer "Log output for X:"
line.
