fix(tools): error-message quality for make/github + post_filter guidance

- make: env-var rejection names blocked vars + target's effective
  allowlist (or deny-by-default) + [tools.make.allowed_env_vars] pointer;
  description documents the mechanism (#59)
- post_filter: overflow guidance adds collection-error patterns; substring
  pitfalls documented; zero-match hint explains sliced-read decoration (#60)
- github: allowlist rejection names tools.github.allowed_operations and
  the yoker.toml opt-in path (#68)
