fix: iterative guardFailures walk — deep results are not phantom 500s (#3160)

The recursive container walk overflowed on deep-but-legal results
(~10k+ nesting) and the RangeError escaped into dispatch's catch as a
phantom function error — the exact hazard the function's getter policy
names and avoids, reopened by its own recursion. The walk now drives
explicit frames on a heap stack (the isJSONSafe precedent), preserving
the seen/cyclic identity contract and rebuild-stands semantics; channel
callbacks already re-enter from fresh async stacks. encodeResult's
codec road also renames residual synchronous throws to encode errors
before rethrow, so misattribution cannot recur from another cause.
