chore: ignore compiled binaries and generated media

Ad-hoc runs of hi264gen/hi264dec leave .mp4/.y4m/.jpg/.264 output and
compiled binaries next to the commands and at the repo root, none of which
were ignored. A working tree had accumulated 733 MB of it across 87
untracked entries, burying real changes in git status noise.

Ignore the binaries (root-level and both cmd/ dirs; only cmd/hi264gen was
covered before) and the generated media, anchored to the repo root and
cmd/ so the tracked assets under images/ and testdata/ are unaffected.
Verified no currently tracked file becomes ignored.
