Ship 0.4.0: Tier 0 production hardening complete

ADR 0016 records the five decisions and the two latent defects they
surfaced. Roadmap Tier 0 marked COMPLETE, stale pre-shipping text
removed. Version 0.4.0; [autopilot] verify documented in both example
configs; CLAUDE.md gains the pg-suite and graph-test conventions (a
graph test that does not fake ingest hits live job boards — that one
cost 97 seconds and real HTTP before it was caught).
