Fix silent delivery outage after channel config migration

When SLACK_CHANNEL and NOTION_DATABASE_ID moved from .env into
karani.toml, five senders (CLI notify/autopilot, MCP notify_slack/
autopilot/notion_sync, the graph's autopilot and notion nodes, Slack
sync command) still read os.getenv directly — each saw empty and
silently skipped, so no Slack cards or Notion updates shipped since the
migration while qualification kept running (hourly log: 'slack not
configured'). Delivery targets now come from canonical helpers
(slackbridge.configured_channel, notionsync.configured_database_id)
with env-over-file precedence intact. Regression tests include a
grep-as-test forbidding raw env reads of delivery targets outside the
helpers. Verified live: digest delivered post-fix.
