docs: Document setup vs demo commands and production deployment workflow

Add comprehensive documentation distinguishing between setup commands (safe for production) and demo seeders (development only). Include a complete deployment checklist for production environments, clarify the purpose of placeholder data in eligibility and water demand seeders, and document the safety implications of running individual demo commands outside of `seed_demo`.
