Release unreached levels' cleanups on startup failure

Wire builds the whole graph before Start runs, so a cleanup's
resources exist even when its level never starts. The rollback walked
only the reached levels and leaked the rest. The teardown pass now
walks every level: an unreached level runs only its cleanups, and its
components receive no lifecycle calls. ADR-015 records the decision;
ADR-003, the architecture document, and the guide state the narrowed
rule.
