[VIZ-001] Break the dashboard builder into VIZ-002..VIZ-009 task designs

Split VIZ-001 into eight per-task design docs in the MT-00x style, four
phases with the backend and frontend tracks running in parallel inside each
phase against the API contract frozen in VIZ-001 section 6.

- VIZ-002 dashboard data model, constants and permissions (BE)
- VIZ-003 visualization endpoint hardening (BE, gated on nothing)
- VIZ-004 dashboard list and create flow (FE)
- VIZ-005 /manage/dashboards CRUD, validation and /sources (BE)
- VIZ-006 dashboard builder UI (FE)
- VIZ-007 publish, snapshot and the read namespace (BE)
- VIZ-008 dashboard viewer and preview (FE)
- VIZ-009 legacy dashboard removal (full-stack, last)

Three changes to VIZ-001 itself:

- D-3 hardened: one form family per dashboard, cross-form permanently
  disallowed rather than deferred, and root_form immutable after create.
  Open question 3 removed accordingly.
- D-10 added: akvo-charts is the only charting library. Widget types map to
  Bar/StackBar, Line/StackLine, Pie/Doughnut and MapCluster; table stays on
  antd since akvo-charts has no table primitive. No direct echarts import.
- Section 12 (AI-assisted generation) extracted into VIZ-AI-001 as its own
  unscheduled epic, out of scope here. The epic ends at VIZ-009.

Adds the interactive builder mockup at doc/design/VIZ-Example, which the
task docs cite by line range.
