Release v0.3.13, and mark the reports whose defects are now fixed

Covers the four recorded defects, the consolidation prepared in report 02, and
the two gaps the T7 experiment exposed.

The reports that documented these defects are snapshots of a point in time, so
their bodies are left exactly as written. Only their status lines change, plus
one follow-up note each, because a report headed 'not yet fixed' describing
behaviour that no longer exists misleads a reader more than a stale snapshot
does.
