Add NAMA 2026 field slip template

The NAMA 2026 foray (Aug 27-30) prints its slips on a DBG-derived
layout that drops the Voucher Number / State / County boxes, widens
the iNaturalist box to iNaturalist/MO, replaces the printed Plants
list with blank Conifer / Deciduous / Other Host lines, and adds
faint DNA and VCP flags in the header corners. A DBG-templated read
would reject these slips as a different form, so they get their own
Template registered for the 2026-NAMA prefix.
