docs: backlog the immaterial gloss and the lens-override panel body

Two items surfaced answering discussion #8, neither belonging to it.

The reference tables gloss `immaterial` as "true and not worth acting on",
which is the verdict's code-review reading. For a filed item verify.md
rereads it as not bearing on the decision, so a triage run's verdicts are
documented under a definition the binary does not use for them.

`--lenses` replaces the roster with one agent but keeps the profile body,
so `--profile triage --lenses cost` runs an agent told three panelists are
working alongside it. Three defensible fixes and no obvious winner, so it
is filed as later rather than done.
