test: tighten ENS world assertions and document realm classification

The ENS non-owner case asserted that LAND ownership at the base parcel does not
grant, but nothing primed LAMBDAS to return a positive permission, so it passed
whether or not the Genesis City fallback was reachable. Prime the LAMBDAS
response to grant and assert the call is never made, so reintroducing the
fallback flips the result and fails the test.

Split the middleware parcel case in two. Its name claimed the world was scoped
rather than the supplied parcel, while the assertion deliberately passes that
parcel through to Places; the two halves now state what each one checks.

Document the realm classification rule in the agent context doc, which still
described worlds only in `.dcl.eth` terms.
