fix(bolt): enforce procedure mode authorization

Authorize registered procedure calls from their declared mode instead of
relying only on outer-query keywords. Propagate effective entitlements to
nested dynamic execution so parameterized statements cannot bypass write,
schema, or admin permissions.

Add Bolt coverage for autocommit and explicit transactions, procedure
modes, and parameterized APOC dynamic statements.
