docs: add cpp-rules.md, the terminal C++ card for execution agents

CODING_RULES.md is observably too broad for the execution agents at the
edges of fan-out. What they need is the C++ file and naming rules, the
component-per-file rule, and the intended shape of the code —
algorithms instead of loops, algebras and catamorphisms instead of
switches, constexpr everything and consteval what cannot be runtime —
but not CMake, and in a form that can be the last thing an agent reads
before writing code.

docs/cpp-rules.md is that card. AGENTS.md Tier 1, CLAUDE.md, and the
R1 brief now route agents to it last.

Consistency patches while here: codestyle.org and CODING_RULES.md now
state the C++26 baseline instead of preferring C++23, and codestyle.org
replaces its Co-Authored-By mandate with the standing prohibition —
that rule keeps falling off because the authoritative style doc
contradicted it.

AGENTS.md's commit policy gains a new rule: commit and PR messages are
reviewed by a clean agent running the voice skill before use; the
reviewer sees only the draft and the diff.

make lint passes. No C++ changed.
