fix(limits): silence macOS/BSD dead_code on post-run evidence helpers

LimitEvidence::new and CappedAxes::has are only called by the Linux and
Windows backends; mark them allow(dead_code) on the POSIX fallback the
same way the existing LimitEvidence::unknown() precedent does, so
--all-features clippy stays clean on macOS/BSD.

Also brings docs/errors.md's stable machine identifiers table back in
sync with src/lib.rs by listing LimitVerdict, and notes that it is a
post-run verdict rather than an admission-time error classification.
