Raise PHPStan to level 6 (#15)

The iterable-value rule is ignored by identifier for now: what remains is
bare `array` needing `array<K, V>`, worked through plugin by plugin. Missing
parameter and return types are gated immediately. The entry is one line to
delete once this plugin reports zero.

Preferred over regenerating the baseline: a baseline would absorb hundreds of
entries and blur the genuine grandfathered findings already recorded there,
whereas an identifier-scoped ignore states exactly what is outstanding and is
removed in one line.

Verified: `composer phpstan` passes against this config.
