BABEL-5975: Handle inline column-level constraints (DEFAULT) in identifier mapping

Extend the CREATE TABLE constraint handler to also iterate
ColumnDef->constraints, catching inline column-level constraints
like DEFAULT that were previously missed. Only top-level table
constraints (from create_stmt->tableElts) were being stored.

This fixes DEFAULT constraint names not being stored in
babelfish_identifier_mapping when defined as:
  val INT CONSTRAINT Very_Long_Name DEFAULT 0
