BABEL-5975: Add upgrade SQL and tests for identifier mapping catalog

Add upgrade script to create babelfish_identifier_mapping table and
recreate constraint/type/sequence/parameter views to resolve original
names from the mapping catalog.

Add focused test file covering:
- PK, UNIQUE, CHECK, FK, DEFAULT constraints with long names
- Sequences and types with long names
- Parameters with long names
- Verification via sys.key_constraints, sys.check_constraints,
  sys.foreign_keys, sys.default_constraints, sys.types,
  sys.all_parameters, sys.sequences
- Drop cleanup verification (entries removed from catalog)
