Remove callback support from bloodhound.track()

track() now always returns a promise. The signature is
track(trackingNumber, [options]); the callback parameter and the
typeof options === 'function' shim are gone, and the one-call-site
executor wrapper is inlined.

Converts all 97 callback-style call sites in the test suite to
await/assert.rejects, and the three README examples. Carrier modules
keep their internal callback interface.

Releases the accumulated breaking changes as 4.0.0.
