fix(security): close CodeQL quoting and SSRF findings

Refactor Cypher internal rewrites to use shared escaped literal helpers,
deny APOC remote URL loads by default with an explicit opt-in gate, and
harden Swagger UI rendering. Add regression coverage and operator docs.
