doc: restructure CHANGELOG

Every entry sat under Unreleased although 1.0.0 had been tagged, so the
file did not describe what any released version contains.
- Move the released entries under a 1.0.0 heading carrying its date.
- Drop the entries for tests, CI and build tooling, which are not
  user-visible.
- Record the handler constructors and the two fixes under Unreleased.
- Link to Keep a Changelog 1.1.0 and add version comparison links.
