Support custom flag prefix from carapace-pflag (e.g. '&' for elvish)

Read the FlagSet prefix character via reflection instead of hardcoding
'-' and '--' throughout pflagfork, traverse, actionFlags, and spec
generation. This enables elvish '&' flag syntax (&&flag, &f, &&)
when the application sets a custom prefix via carapace-pflag's
FlagSet.SetPrefix().
