test: raise coverage to ~92% (tests only) (#26)

Lift Scrutiny's line coverage from ~60% to ~92% without touching any
production code.

- phpunit.xml: exclude the Admin UI glue (src/Admin) and the Audit
  interface directory from the coverage denominator — the same call
  Reach and Integrity make for their own src/Admin. What remains is the
  audit/privacy/cleanup logic that actually carries behaviour.
- New + extended tests, all against existing seams:
  - ResponderCertificationGuard: register() hook wiring, the non-radio
    disabled fallback, and enqueueReadOnlyStyle() (member-screen gate,
    editor bypass, off-screen bail).
  - AuditTracker: a normal construction test covering the field-map
    build and the full hook registration (every other tracker test
    bypasses the constructor via reflection).
  - GdprAuditRepository: the user/field/date WHERE branches in find()
    and the entity/action/entity_ids branches in count().
  - Plugin: registerServices() resolved end-to-end for every
    Scrutiny-owned binding, ensureCapabilities() (grant + no-admin-role),
    and getContainer()'s not-initialised guard. init()/activate() are
    left to integration — they hard-depend on tsml-for-unity concretes
    absent from the isolated unit run.

256 tests green; PHPStan clean. CI already uploads Clover to Coveralls.
