fix(notify): surface banner suppression in the log and the control response

notify answered a bare `ok` and logged nothing when "Show notification
banners" was off, so a call that never reached macOS was indistinguishable
from a broken notification path. The documented `log show` recipe returned
an empty result for a healthy notify too, since the success path logged
nothing either.

- log every posted notification and every suppression (the banner toggle in
  both paths, plus OSC focus-suppression) at .notice, the level `log show`
  persists
- return the shared ControlNotify.bannersOffNote in result.text when banners
  are off, mirroring the session.restore note; the CLI prints it in place of
  `ok` with no CLI change
- correct docs/troubleshooting.md and README.md, which pointed at
  "Settings > General > Notifications" - that tab no longer exists, the
  toggle is on the Notifications tab
- document the response contract in the agent skill and site/commands.html
