add custom flag prefix support to FlagSet for non-dash prefixes

Allows configuring a custom flag prefix character (e.g. '&' for elvish)
via FlagSet.SetPrefix(). The prefix controls token recognition, long/short
disambiguation, terminator detection, error messages, and usage output.
All structured errors carry a prefix rune field so they render correctly
with the configured prefix. Zero-value FlagSet defaults to '-'.
