fix: Don't truncate base64/base32 CLI output

fix: Streaming encoders were never closed, dropping the final partial
group (a 20-byte SHA1 became 24 base64 chars, not 28). Use
EncodeToString instead and add a decode round-trip test.
docs: Fix three stale README digests.
