feat: Add options to ignore inputs and variables

Introduce parser.Options with IgnoreInputs and IgnoreVariables so a
document can be hashed by structure while ignoring its input data:

- IgnoreInputs ignores every argument/default value (scalars, lists,
  input objects and variable usages all collapse) while keeping the
  query structure and variable signature.
- IgnoreVariables is a superset that additionally drops variable
  definitions.

Expose them via the -ignore-inputs and -ignore-variables CLI flags and
the AppendQueryHashWithOptions and ReadDocumentWithOptions functions.
