test: raise coverage to ~92% (tests only) (#23)

Lift Unity's line coverage from ~20% to ~92% without touching any
production code — the eight concrete classes (the container, config
store, cache adapter, service provider, logger trait, exception, the
ResponderCertification enum and the Plugin bootstrap) now sit at 100%
method coverage.

- phpunit.xml: exclude the pure-interface directories from coverage.
  They carry no executable behaviour — only the
  `if (!defined('ABSPATH')) exit;` guard — so they have nothing to
  unit-test and only distort the denominator, exactly as Reach and
  Integrity exclude their src/Admin render glue. The remaining ~8%
  is those unreachable exit guards in the concrete files.
- tests/bootstrap.php: add a Sentinel_Log_Channel stub so the HasLogger
  path can be exercised.
- New tests: UnityConfiguration, WordPressCache (WP_Mock wp_cache_*),
  UnityServiceProvider, DependencyNotRegisteredException,
  ResponderCertification, HasLogger, and the Plugin static façade
  (init / initContainer / initServices / getContainer / getInstance /
  setInstance / deactivate) plus tracker resolution and its guards.
- .gitignore: ignore the local tests/coverage HTML report.

47 tests green; PHPStan clean. CI already uploads Clover to Coveralls.
