Reduce responder-certification to five stages (#21)

Collapse the certification stages to None, Applied, In Training, Pending,
Certified. Rename 'Certification Pending' to 'Pending' and drop the
'Recertification Required' and 'Denied' stages, updating both the enum
and the ACF radio choices in unity_v1.json so the stored value and the
case values stay in lockstep.

No data migration ships with this: a member holding one of the removed/
renamed raw values resolves to None via fromAcfValue(), as documented.
