Consume backend list_changed in vMCP and propagate to clients (#5965)

vMCP registered no persistent notification handler on its per-session
backend connections, so backend tools/list_changed was dropped and the
aggregated view went stale until the short-TTL cache expired. Register a
persistent, non-blocking, coalescing handler that invalidates the cache
and re-syncs the session's tool set (authenticated as the principal),
letting the per-session go-sdk server auto-emit tools/list_changed to the
client. Flip WithToolCapabilities(true). notifications/message is
log-only.
