Propagate W3C trace context in outbound MCP _meta (#5964)

Wire the vMCP protocol-terminating client to inject the active W3C trace
context (traceparent/tracestate/baggage) into outbound params._meta per
SEP-414, so client -> vMCP -> backend traces join for MCP 2026-07-28
peers. Backward compatible: no _meta is emitted when there is no active
trace context.

Closes #5758
