Retain debuggable v2 failure artifacts (#186)

* Retain v2 planner state on failure

Keep streaming v2 planner-state scratch directories on disk when planning or finalization exits through an error, update the streaming-indexer spec package, and add executable validation coverage.

Strengthen the streaming v2 failure retention contract so preserved planner-state roots carry bounded deterministic summary and detail artifacts that explain replay/classifier empty-child failures.

Use TempDir::keep() when preserving planner state roots and replace eager full-trainer debug dumps with bounded structured classifier routing summaries so successful runs do not pay a large formatting cost.

Preserve the original failure error when summary emission fails, only reference a detail artifact when it exists, harden retained-root assertions in the validation tests, and trim the replay-order child count cloning noted in review.

* Preserve summary-write context on v2 failures

Keep the original v2 failure variant when failure-summary emission breaks, but append the summary-write error into string-backed variants so missing retained artifacts are surfaced to callers without regressing the public error shape.
