Harden add-model per review: bad-catalog, ignored flags, EOF

- Validate the picked provider has base_url + api_key_ref and skip catalog
  models lacking an id, so a malformed (LLM-refreshed) catalog yields a clean
  error instead of a KeyError traceback.
- In interactive mode, honor --name and --pool as the prompt defaults instead
  of silently ignoring them.
- Catch EOFError/KeyboardInterrupt at the CLI top level (e.g. non-tty stdin) and
  exit 1 with "aborted" rather than a traceback.

Extracts the file-append half into _append_custom_entry (keeps the command
under the complexity limit). Adds tests for each fix.
