Add implementation plan for the add-model command

Plan for an interactive CLI that picks a paid provider/model from a
git-fetched presets/paid-catalog.toml and appends a [[custom]] entry to the
user's config: git-fetch reuse, append-only comment-preserving writes,
pool=false default, interactive + flag modes, and the test matrix.
