Migrate CI from Semaphore to GitHub Actions

Replace .semaphore/semaphore.yml with .github/workflows/ci.yml, using
erlef/setup-beam across a matrix of Elixir 1.16.0/OTP 26.2 and Elixir
1.20.1/OTP 27.2 (per the official compatibility table), with dep/build
caching and a concurrency group to auto-cancel superseded runs.

Point bin/test's coverage upload at mix coveralls.github instead of
coveralls.semaphore, and swap the equivalent (and by now dead)
coveralls.travis entry in mix.exs's preferred_envs for coveralls.github.

Update the README build badge to point at this fork's Actions workflow
instead of upstream's Semaphore project.

Note: mix coveralls.github needs this fork enabled via Coveralls' GitHub
Actions integration for the coverage upload to succeed.
